Each family who participates in the Moon Festival meeting will need to complete this form.  This is often required when we rent other facilities.  If your child is bringing a guest with them to the Moon Festival, please print out this form and have the guest's parent complete it.  Make sure you remember to bring the completed form to the Moon Festival so that your child's guest can participate in the activities.
